POSITION SUMMARY

 

As a member of the care team, the Social Worker develops care plans to address patient's social determinants of health and assists patients with community resources to help with financial, social, spiritual, mental, and physical needs.

KEY RESPONSIBILITIES

  1. Establishes rapport and working relationship with patients in order to develop care plans for individual patients to best meet patient's needs.
  2. Interviews patients to assess specific needs, arranges for appropriate interventions, and completes documentation that meets CCHS standards as well as the requirements of outside agencies.
  3. Addresses barriers to care such as access to transportation, low literacy, and difficulty paying for medication.
  4. Serves as a liaison for home health, physical therapy, occupational therapy and speech therapy.
  5. Coordinates referrals to behavioral health and substance abuse services.
  6. Conducts prior authorizations for procedures or tests and orders durable medical equipment.
  7. Assists with appointment reminder calls for patients, as needed.
  8. Addresses clients' social determinants of health. These are the conditions in the places where people live, learn, work, and play that affect a wide range of health risks and outcomes.
  9. Collaborates with various outside agencies to assist patients with receiving benefits such as food stamps and TennCare, advocating for patients' rights, as necessary.
  10. Assists uninsured and underinsured patients with obtaining medications.
  11. Completes care plans with patients regularly to assess current status and progress.
  12. Report child abuse, domestic abuse and elder abuse.
  13. Performs other duties as assigned.

POSITION REQUIREMENTS

Education: Bachelor's degree in social work

 

Experience: One to two years working with patients in some type of healthcare setting and a working knowledge of basic medical terms.

 

Licenses or Certifications: N/A

Level 1 – Requires some concentration and normal attention. Generally, once the job is learned, the tasks can be performed more or less automatically.

 

Level 2 – Requires high periods of concentration intermittently and normal attention. Generally, even once the job is learned, tasks will require normal attention to deal with recurring variables.
